WTP Potential Donor Personal Request

Please provide us with the following information so that we can generate a customized appeal for year-end,
tax deductible donations for the WTP Foundation.  Shortly after we receive your information, we will send the
potential donor that you have identified a full-color WTP informational brochure and a personalized letter about the WTP organization and our mission.

The customized letter will identify you as the referring individual and may provide minimal contact information
so the potential donor can contact you directly for additional information about WTP or inquire about
your support of, or experiences with WTP.  See a sample letter.

We would strongly urge you to follow up our mailing with a personal phone call to the potential donor call so that donations can be arranged in time to take advantage of the tax deduction for this year.

Please fill in the information below as it will appear in the generated letter for the potential donor.
Please remember to Capitalize (only) the first letter of all Names, Cities, Titles, etc.
PLEASE be sure to check your spelling!

Please be judicious in your requests as to the number of requests and potential donors that you are personally referring.  We will honor as many of these requests as we can afford to.
